The Foundation for Better Health (formerly Tom Green County Partnership for Better Health) is part of a state initiative to reduce potentially preventable hospitalizations. The Department of State Health Services has contracted with Tom Green County through the Foundation for Better Health to implement evidence-based interventions, through a community coordinated approach, to reduce hospitalizations and/or hospital charges among adult county residents for Bacterial Pneumonia, Urinary Tract Infections and Chronic Obstructive Pulmonary Disease. The purpose of the initiative is to assist in improving health care and reducing health care costs for Tom Green County residents.

The Texas Department of State Health Services estimates that the combined efforts of all of the state partnerships would result in approximately 6,450 fewer hospitalizations, 11.9% fewer hospitalizations charged to the uninsured, 32.1% fewer hospitalizations charged to Medicaid, and $230.9M in avoided hospital charges on a biennial (two-year) basis.

Med Bag Initiative

Med Bag Initiative

The med bag initiative is a campaign designed to raise awareness about taking your medications properly and potentially reducing the number of medications that you need to take. We are encouraging patients to bring all medications, vitamins, and supplements with them to all doctor’s appointments. As a part of this campaign, the Foundation for Better Health will be providing medicine bags that are free to the public. These bags are perfect for safely storing and keeping track of all your medications for easy transport to your next appointment.

Flu vaccination can keep you from getting sick from flu. Protecting yourself from flu also protects the people around you who are more vulnerable to serious flu illness. It also helps protect people who are at greater risk of getting seriously ill from flu, like older adults, people with chronic health conditions and young children (especially infants younger than 6 months old who are too young to get vaccinated). Flu vaccination can also make your illness milder if you do get sick and also reduce your risk of more serious flu outcomes such as hospitalization.


Prevention is the key when it comes to reducing your risk hospitalization. Recognizing and managing the symptoms of preventable disease allows patients to seek medical attention before hospitalization is required. On the other hand, preventing disease altogether is a much better alternative. Knowing the steps and lifestyle changes that will help reduce your risk of preventable disease will not only reduce your risk of hospitalization, but will also improve your quality of life overall.

Preventable hospitalization is also very expensive. From 2005-2009, adult residents (18+) of Texas received $31,926,978,628 in charges for hospitalizations that were potentially preventable; $159,959,609 of those charges were in Tom Green County alone. Hospitalizations for such conditions are called ‚Äúpotentially preventable‚ÄĚ because if the individual had access to and cooperated with appropriate outpatient healthcare, or had taken the proper steps to help prevent the disease in the first place, the hospitalization would likely not have occurred.
